1. Product model number (including serial number for tuners). 2. Clear pictures of the item/device (front and back with readable serial or product code). Not applicable to those who wish to have tunes loaded to their device. 3. Valid proof of purchase either a scanned or photo receipt or screen shot of online purchase or purchase order number.
